Dear Parents/Guardians:
As you are probably aware, all high school student athletes are required to have a yearly pre-participation physical exam. All student athletes are required to have a physical examination and return the completed physical exam form to the Athletic Department before their first Summer practices by JUNE 12, 2023.
The Armens Athletic Department will be having Doctors to come to Holy Martyrs Ferrahian high school on May 26, 2022 at 3:45 PM to examine our athletes for Physical examination. The cost for the Physical examination is $35 per athlete. Please sign up as soon as possible. The sign up due date is Thursday May 12, 2022.
DEAR ARMENS PARENTS AND ATHLETES
PARENTS IN ORDER FOR YOUR CHILD TO PLAY ON ANY ARMENS TEAMS FOR THE UPCOMING 2022/2023 SEASON THEY NEED TO HAVE THEIR SPORTS PHYSICAL TAKEN CARE BEFORE THE SUMMER PRACTICES. THEY WILL NOT BE ABLE TO PRACTICE WITHOUT THE PHYSICAL EXAMINATION SIGNED AND STAMPED BY THEIR DOCTOR.

Please be sure to completely fill out your portion of the examination form before your child sees the doctor (first page is the history form). The form can be found here below.
