Dear Parents/Guardians:
As you are probably aware, all high school student athletes are required to have a yearly pre-participation physical exam. All student athletes are required to have a physical examination and return he completed physical exam form by Sunday June 8, 2014.
For your convenience, we are once again pleased to announce that we will be offering sports physicals at the Ferrahian Encino Campus. The on campus exams will take place on Wednesday, May 14, 2014right after school at 3:30PM.
The cost of the physical is $30. The exams will be performed by community doctors and nurses who have kindly agreed to volunteer their time. All of the money collected will benefit the Armens athletic program.
We highly encourage you to participate in this endeavor and help support our school. We very much appreciate the time and talents of our volunteers and we hope to have a good turnout. Should you wish to participate, please reserve a spot by scheduling an appointment with coach Varant by Friday, May 9th. We kindly ask that you also pay for the cost of the physical when making your appointment so we may reserve your spot. Payments may be made by cash or check (“Ferrahian Armens”).
Please be sure to completely fill out your portion of the examination form before your child sees the doctor (first page is the history form). The form can be found online on our website at Ferrahiansports.com.
