Dear Parents/Guardians,

For the safety of our students, all children are expected to be picked up directly from school by a parent/guardian or an authorized adult unless written consent is provided. If you would like your child to walk home from school on their own, please complete and sign the form below.

By signing this consent form, you acknowledge and agree to the following:

1) I, the undersigned parent/guardian, give permission for my child, (enter student’s full name), to walk home unaccompanied after school dismissal.*

2) I understand that once my child leaves the school premises, the school is no longer responsible for their safety and well-being.

3) I accept full responsibility for my child after dismissal and release Holy Martyrs Ferrahian High School, its administration, faculty, and staff from all liability for incidents, accidents, or harm that may occur after my child has left the school grounds.

4) I agree that this consent remains in effect for the school year 2025–2026, unless I notify the school in writing to revoke my permission.
